Open Letter
By Lorraine Evans

The biggest achievement for TVT Mum was finding help and giving emotional support to all those suffering serious health issues since having the synthetic mesh implant medical device surgeries. I have helped over a thousand women across the whole of the UK, USA, NZ and other Countries where they can contact each other. I have provided information and pointed them to World leading surgeons experienced in synthetic mesh complications this was achieved through the support group sharing their surgery experiences and giving emotional support for one another.

On reflection I can see the vast majority of people I have met during those years are the most wonderful and caring people and very proud to say, “We did it! Our voices are being heard throughout the World. We have supported each other for years as unpaid volunteers, we can look at the past and the forthcoming years to see what we have achieved and sometimes it appeared to be the impossible feat but we did it with sheer spiritual strength, believing we can win this mesh journey and most importantly we can have our life back and to know there is human kindness to help us through”

Please know I haven't left the TVT Messed up Mesh (TVT Mum) charity and won't ever give up working on the website to bring the latest news plus I will continue to provide a support, help and advice help-line to those who need it.

Be happy, kind to each other and to yourself. If there is an opportunity to help someone going through a difficult time help them whether it’s by invite, sharing experiences, and above all to show empathy for one another.

Presently I am so much happier in my life I have made the decision to move on for better times ahead.

Wishing you all with Health, Wealth and Happiness for the New Year 2014

Victory for the Mail on surgery that left thousands in agony: Hospitals ordered to warn patients over incontinence operation that can go cruelly wrong
Before Karen Russell would agree to an operation in 2007 to remedy the incontinence she had suffered since having babies, she wanted assurance that the procedure was low-risk, and that she would have no problems walking her two children to and from school.
‘The surgeon told me it would be uncomplicated, with no major risks,’ recalls Karen, 44, a housewife from Peterborough.
But, in fact, the op, which used a treatment called transvaginal tape (TVT) — a mesh implanted under the bladder like a hammock to treat incontinence by supporting the internal organs — had such terrible consequences that she was left in agony and used a wheelchair for five years.

Revealed: Shocking figures reveal that hundreds of women have been affected by mesh implant scandal
By Marion Scott

POLITICIANS joined victims and experts yesterday to demand the immediate suspension of mesh implants as figures reveal hundreds of women suffered horrific side effects – not just six as ministers claimed.
The call comes as lawyers prepare to lodge the biggest medical claim in Scottish legal history, with 400 victims set to sue the implant manufacturers and the NHS.
Pressure was mounting on Health Secretary Alex Neil over the scandal – first exposed by the Sunday Mail in March – after his officials were accused of understating the number of women who endured terrible pain caused by the transvaginal mesh.

Women angry over side-effects from another Johnson and Johnson product
ABC News Australia
Stephanie Smail reported this story on Monday, November 25, 2013
MARK COLVIN: Hundreds of women have joined a class action against the medical giant Johnson and Johnson over a range of their products.
They say they've suffered devastating side-effects from products designed to support weak pelvic floor muscles.
It follows other class actions over the last three years over Johnson and Johnson's joint replacement systems.
In the latest case, some of the products used on the women have been discontinued, but others are still available.

Revealed: Scots doctors were told to warn patients about mesh surgery risks four years ago
IT has emerged that NHS Scotland were warned of the risks of the controversial surgery in 2009. HEALTH bosses were warned four years ago that patients were not being fully informed of all the risks of controversial mesh surgery.
It has emerged that Scotland’s biggest health authority, NHS Glasgow, were rapped by the Public Services Ombudsman and ordered to review consent processes in 2009.
The warning can be revealed weeks after Health Secretary Alex Neil ordered NHS officials to rewrite consent forms to include information about problems associated with mesh. He said every woman should be given “all the information they need to make informed choices”.

Message from Lorraine Evans
TVT Mum is a registered not-for-profit charity in England and Wales. We focus on providing emotional help and support with useful information to our members pointing them in the right direction to speak with expert medical professionals in mesh related complications. We also bring awareness on the serious health concerns from vaginal meshes to professional organizations such as the Medicines and Healthcare Regulary Agency (MHRA).

I decided to take some time-out this year after working voluntarily since 2008 on research, bringing awareness on the serious health issues from vaginal meshes, meeting with various organizations and providing help and support to our members.

Our awareness work continues and if I am asked what is happening concerning meshes I often tell people the serious concerns on vaginal meshes is in the hands of medical and regulatory organizations. We believe it will take possibly years for a review and for an effective implementation on the medical device regulatory system outlined through our awareness work.

Mesh Sling in an Era of Uncertainty: Lessons Learned and the Way Forward

Christopher R. Chapple, Shlomo Raz, Linda Brubaker and Philippe E. Zimmern
Published online 4 July 2013
European Association of Urology

For many years, synthetic mesh was avoided whenever possible for surgical treatment of stress urinary incontinence (SUI) and/or pelvic organ prolapse (POP) because of the recognized complications of fibrosis and erosion seen with Mersilene [1] and Gore-Tex slings [2].

Petros and Ulmsten [3] in 1990 and Petros and Papadimitriou [4] more recently described a fairly simple procedure with tension-free vaginal tape (TVT), during which the surgeon placed a thin strap of polypropylene mesh in a midurethral position. Since the 1990s, the marketing and use of synthetic materials for SUI and POP indications have dramatically increased. This was particularly noticeable after the publication of a randomized controlled trial comparing TVT with colposuspension [5]. A number of similar procedures were subsequently granted marketing licenses with little clinical data from adequately powered randomized studies. This was followed by a series of modifications including transobturator tape (TOT) [6] and, recently, a wave of single-incision slings, or mini-slings, to prevent passage of trocars through the retropubic space or obturator fossa [7]. Concomitantly, the specialty of female pelvic medicine and reconstructive surgery has witnessed the very rapid growth of larger segments of synthetic material, referred to as mesh, being implanted beneath the vaginal wall to correct POP based on the early data supporting efficacy of TVT and TOT.

More than 40 implants are on the market [8] and [9] and are used with little evidence related to mid- and long-term safety and efficacy. Training to place these new implants has often comprised cadaver courses on weekends, review of video procedures, observing “experts” performing implants, and mentorship in institutions by a visiting surgeon. The use of these materials and the surgical techniques have not been limited to subspecialist practice. In 2008, following an escalation in complications reported to the Manufacturer and User Facility Device Experience (MAUDE) database, the US Food and Drug Administration (FDA) issued a first notification to inform the public [8] that these devices and “kits” had risks, should be used with caution, and might result in nonreversible outcomes [10]. A second FDA notification in 2011 sounded even more alarming [11] and provoked a chain reaction from patients, physicians, manufacturers, and lawyers. Similar initiatives were under way in the United Kingdom, with recognition of the problem by the Medicines and Healthcare Products Regulatory Agency (MHRA) [12] and [13]. As the Internet facilitated connection between desperate patients seeking help [14], television advertisements started to inform the public about issues related to “transvaginal meshes.” A number of Web sites inspired by patients’ experiences identified problems with mesh (eg, TVT Messed up Mesh [TVT Mum], http://www.tvt-messed-up-mesh.org.uk/).

During specialty meetings, many presentations and discussions have focused on mesh or tape complications and their management, specifically, obstruction, pain, dyspareunia, and erosion that may have irreversible consequences despite multiple interventions [10]. In daily practice, patients have begun to inquire more intensely about “mesh” or “tape,” and the regulatory authorities have provided information for patients on this subject. There is a lack of registries to establish the true incidence of the problems with the use of synthetic materials, as has been recognized with the underreporting of these complicated cases to the “optional” MAUDE database [15] and to the other regulatory bodies such as MHRA in the United Kingdom. Although voluntary registries have been established by professional groups, they do not provide accurate information because registration of all cases would be required and, invariably, existing voluntary registries are prone to selective reporting of cases. Midurethral tape surgery for incontinence; a possible victim of the vaginal mesh crisis?

12 Apr 2013
incontinence, Marcus Drake, surgery
by Marcus Drake

Type 1 mesh is used in vaginal surgery for pelvic organ prolapse repair, along with the mid-urethral tapes for stress incontinence surgery. Tapes for incontinence surgery are well-established and systematic review shows that retropubic tape is probably more effective than colposuspension, risk of bladder perforation notwithstanding [1]. The various types of mid-urethral tape appear to have broadly equivalent efficacy, but the poor quality evidence-base is an issue. The real problem lies with the major complications that can occur, some of which are highlighted in the recent statement from the US Food & Drug Administration in response to concerns expressed by patients and other stakeholders. Mid-urethral tape itself is recognised to be at risk of important complications in the long term, and mesh exposure in the vagina is a major issue with considerable detrimental impact. Patient groups have become organised in recognition of this and they are setting up online dialogues and websites accordingly, for example “tvt-messed-up-mesh.org”. The surgical professions have to agree how best to manage the difficult problems, dealing with the exposed mesh and handling the further procedures needed to re-establish continence [2].

Labi Siffre - Something Inside So Strong
Lyrics below the video

The higher you build your barriers
The taller I become
The farther you take my rights away
The faster I will run
You can deny me
You can decide to turn your face away
No matter, cos there's....

Something inside so strong
I know that I can make it
Tho' you're doing me wrong, so wrong
You thought that my pride was gone
Oh no, something inside so strong
Oh oh oh oh oh something inside so strong

The more you refuse to hear my voice
The louder I will sing
You hide behind walls of Jericho
Your lies will come tumbling
Deny my place in time
You squander wealth that's mine
My light will shine so brightly
It will blind you
Cos there's......

Something inside so strong
I know that I can make it
Tho' you're doing me wrong, so wrong
You thought that my pride was gone
Oh no, something inside so strong
Oh oh oh oh oh something inside so strong

Brothers and sisters
When they insist we're just not good enough
When we know better
Just look 'em in the eyes and say
I'm gonna do it anyway
I'm gonna do it anyway

Something inside so strong
And I know that I can make it
Tho' you're doing me wrong, so wrong
You thought that my pride was gone
Oh no, something inside so strong
Oh oh oh oh oh something inside so strong

Brothers and sisters
When they insist we're just good not enough
When we know better
Just look 'em in the eyes and say
I'm gonna do it anyway
I'm gonna do it anyway
I'm gonna do it anyway
I'm gonna do it anyway

Because there's something inside so strong
And I know that I can make it
Tho' you're doing me, so wrong
You thought that my pride was gone
Oh no, something inside so strong
Oh oh oh oh oh something inside so strong

Because there's something inside so strong
And I know that I can make it
Tho' you're doing me, so wrong You thought that my pride was gone
Oh no, something inside so strong

Please tune in or record to hear about the aftermath of whistleblowing for many care workers and hear me talk about how I was banned from a Cornwall Care Ltd home for 30 months after I complained repeatedly about the same issues. Serious allegations were fabricated by care staff and senior management about me to keep me out of the care home and attempts were made to stop me seeing my loved one altogether which were rejected by Adult Safeguarding. Thanks again to the care workers who agreed to be interviewed for this feature.
